Coos Health and Wellness has created a form for those interested in getting the COVID-19 Vaccination. Front desk staff at Coos Health and Wellness have been overwhelmed by calls regarding and registering to receive the vaccination. CHW Officials say currently the only way to get an appointment for the vaccination is by filling out the form. They also stated that based on the information they are able to target groups and notify those eligible where and when they can get the vaccination. Last weekend, a vaccine clinic was held at the Coos Bay Fire Station and over 400 people received the vaccine. To fill out the form visit cooshealthandwellness.org
